Agreement and Release of Liability Standard Waiver
In consideration of being
allowed to participate in the yoga classes with Christine Germyn, I
hereby waive, release and forever discharge
Christine Germyn and resort property, employees, representatives,
executors and all others from any and all responsibilities or
liability from injuries whether physical or mental, accident, death,
delay or inconvenience to the customer or any other person whatsoever
to delay or damages resulting from my participation in any of the yoga
activities.
I also hereby release all
of those mentioned of any injury or damage to myself, including those
caused by the negligent act or omission of any of those mentioned or
others acting on their behalf or in any way arising out of or
connected with my participation in any yoga activities with Christine
Germyn.
I understand that yoga is
a physical practice therefore my health is in good condition and I am
at my own risk if I injure myself.
I hereby agree to expressly assume and accept any and all risks of
injury or death. I do hereby further declare myself to be physically
sound and suffering from no condition, impairment, disease or other
illness that would prevent my participation.
If you have any
limitations such as; heart condition, high blood pressure, any recent
injuries or if your pregnant this may not be suitable for you. Please
consult your physician prior to the yoga retreat.
